ENROLLMENT INFORMATION

Thank you for your interest in Pontiac Academy for Excellence (PAE). We are excited about the educational opportunities PAE will be able to offer the students.

To be considered for admission to Pontiac Academy for Excellence, all applicants must complete the following steps:

  • Complete the online enrollment application

  • The following documents needs to be submitted with each application:

    1. A copy of the applicant’s birth certificate (original).

    2. A complete health report (Kindergarten applicants only) completed by a medical personnel which includes vision and hearing screening.

    3. An updated/most recent immunization record.

    4. A copy of parent/guardian ID.

    5. A copy of applicant’s last report card (1st thru 8th grade)

    6. A copy of transcripts (high school students only)

    7. Completed Affirmation of Prior Discipline Record form.

    8. A copy of any special education information (if services were received)

    9. Completed emergency card

    10. Sign the student handbook agreement

    11. Sign the Family Educational Rights and Privacy Act and Directory Information

    12. Complete the marketing parent consent form.

    13. Complete the Consent for Disclosure of Personally Identifiable Information and Immunization Information.

    14. Complete the permission to release official records form.

SPECIAL NOTE: Applications will be considered incomplete unless all of the above items are included.
